hi ~Cris Neoh , Based on our checking from our side, we notice that all the setup for this promotion related tot this sales order are made in good manner.
from our side we suspect two possibilities:
1) the promotion could be disappeared during admin approve process
2) the promo could be miss when the admin do modification on that particular sales order.
Further action that has to be done:
1) for temporary workaround
~Joe requested to do patching the on the promotion on that particular product
2) Simulation
3) Escalate to technical team with the simulation finding.

Tomorrow Patching has to be done tomorrow
